- Please bring your insurance information with you. We try our best to minimize the hassle of billing your insurance.
- Insurance covers some but not all of the costs of your treatment; co-payments and deductibles are also common to most insurance. You are responsible for these payments at the time of your visit.
- We send the claims to your insurance on your behalf, as a courtesy to our valued patient. However, if your insurance company fails to cover the payment in 45 days, you will be responsible for the payment in full upon receipt of statement of account from us.
- Any further required action, correspondence or discussion with your insurance company, or furnishing of any documentations needed by your insurance company will be your responsibility.
